We will be able to pick up children from a limited number of schools and bring them to Makom Afterschool, either on foot or by TTC. Transportation provision costs an extra $650 for the year. We will prioritize families registered by June 1st and those routes with most students, so sign up early and spread the word!

I give permission for my child to participate in Makom field trips. I understand that transportation will be either by carpool, school bus, TTC, or by foot, as appropriate.

I give permission to Makom to photograph or videotape my child during program hours or at any program-related activity, and to use such photographs in program communications or publicity of any kind, including but not limited to newsletters, event flyers and on Makom’s website and social media such as Facebook. I understand that my permission means that Makom is best able to publicize Makom Afterschool to potential students and others. Permission does not include use of my child’s photograph for any purpose other than to communicate information about Makom. I understand that my child’s name will not be published unless I am explicitly asked for additional consent.

I give permission to Makom Afterschool to share my name and contact information as well as my child's name and photo for the express purpose of administrative oversight of Makom Afterschool (for example, to companies that provide technology to track attendance and/or manage school-parent communications). I understand that my permission means that Makom is best able to administer Makom Afterschool safely, as well as to meet the licensing requirements of Ontario's Ministry of Education. My permission does not include use of my child’s name and photograph for any purpose other than to communicate information between the school's staff and myself.

I give permission for my email and phone number to be shared with other Makom Afterschool parents.

Further, I understand that subsequent payments become non-refundable after their due dates.
